Description

Problem: User ID A0160 has been deleted from the system because inactive. On 9/11/12, IS received application form for re-activation. But creation failed – message: Username already exists.

Prod_013 - Description: Delete user ID but cannot create new for this user
User had been deleted and create new for this user but error - "user already exist"

Root Cause: Invalid Table Name
Run script to update table name
