Description
- To display pop up instead of reloading in the same page for Add new Sub Activity.
- To set a Date limit based on main event date
- To add City drop down field

Hi Chee Onn,
Reported issue tested but found a bugs during create new Sub Activity.
Step to reproduce
1. Click on '+' sign to create new sub activity
2. Create Form display
3. Click on 'Cancel' button
Actual Result: Click on Cancel button system reload Sub Activity List.
Expected Result: Click on Cancel button will close the window directly
