Got the reply from Bank on 14th OCt 2016. Conversation can be tracked in the email with titled "RE: CAS User Profile".

Remarks :
1. Suspect error due to user information
- Information provided by Bank on 20th and 27th September 2016.
- Found no data issue for the user and application access, no error caught in the log file.

2. Suspect error due to token data (more than 1 token tied to user)
- Information provided by Bank on 14th October 2016.
- Found that the there is 2 tokens tied to the user. System expected to get single token data from database, however database returned with 2 token data. and caused the error.

3. Now pending Bank to confirm and verify with Business user, to revoke the another token which not used by the user. Last reply on 14th Oct 2016
