Please attend customer query below:
Upon checking the below transaction, noticed that there's only 1 successful IBG-OTC transaction (Ref#065604) performed by JB branch however it was reflected as 2 successful entries at Corus. Resulted we have to recall one of the transaction from Bene Bank. According to Law Nee, they encountered system (GFS) hang after performing the overwrite for the 1st attempt (Ref#065603). Upon checking at Phx, the entry has been reverted due to system failure however this trx was reflected as successful at Corus.

found the the problem and fix it..
sent to IT to deploy into staging for user test
the problem because status is not set to failed(F) when system revert the transaction
