Issue: Prompt message Fail Falsed when user want to submit form for new activity (refer attachment)

Finding:
Issue occurred due to fixes on Support #5901 (Fixes only for edit). Create and save new activity did not save all details, thus throw error failed to save.
Solution:
Fix code to save all new activity details.
Test scenario:
1. Access Planning > Add Activity.
2. Create new activity.
3. Fill in all fields.
4. Submit. Expected result: Activity can be saved without error.
5. Find the new activity and review activity details from VIEW or EDIT. Expected result: Activity details should be the same as when creating activity.
